3. Birth Certificate Registration Time Limit:
Normal – Within 21 days of birth occurrence.
With Penalty – If the birth is not registered within 21 days, it should be registered within 1 year with some nominal late fee.
After 1 Year – If the birth is not registered within 1 year, it should be registered by informing the magistrate or executive magistrate along the affidavit. Magistrate verifies the detail and makes an order to the registrar to register.
4. Responsible Person to Register the Birth of a Child:
- If child born at home – Head of the family is responsible / Head of the Home / Nearby Relative / Oldest family member can inform or register the child birth.
- If child birth happened outside the house –
- In Hospitals – Hospital In charge, Officers In charge, Deputy Superintendent or concerned authority.
- In Maternity Home – Medical Officer or Maternity home officer in charge.
- In flights – Flight In charge or concerned ground authorities.
- In Vehicles – Vehicle In charge.
- In Jail – Warden In charge.
- Dharamshalas/Pilgrimage sites – In charge of Dharamshalas or In charge of Pilgrimage Sites.
- Other Places – Responsible person of concerned places.
5. Documents Required:
In Hospital – Hospital discharge card.
Outside Hospital –
Informers ID should be provided (Any Government Authorized Identity Proof).
Informer’s confirmation letter.
Letter of Affidavit.

7. How to apply for birth certificate online in Uttar Pradesh?
Step1: Visit Birth and Death Registration Portal
Step 2: Login in to the portal or Select “General Public Signup” if you are first time user.
Step 3: Fill in the details like user name, user email id, mobile number, date of occurrence of event, state/UT, district, sub district/taluk, village/town, registration unit and type the security captcha. Then click “Register”.
Step 4: You will be receiving a message of “Thank you for registering” and asked to activate the user ID by checking email inbox.
Step 5: Copy the Login Id received in your email Id and Click “Please click here to activate your account”.
Step 6: You will redirected to login page, in the page paste the login Id and create a strong password for that account. Enter captcha and click “Submit”.
Step 7: Now the account is activated. Please re-login to experience the services.
Step 8: In the “Birth” menu click “Add birth registration” in the drop down.
Step 9: You will redirected to Birth Registration Form. Please fill up all the necessary details.

In Hospitals – It’s easy to get the birth certificate if the child is born in the hospital. Please submit the necessary documents to the hospital and register it to the concerned authorities.
